They also have a website where you can check what you need depending on destination:

Regarding the Vietnam visa, best to do that in Bangkok or Phnom Penh if you're passing through that way.

It costs about about $50 in Bangkok but can be a bit more depending on how fast you need it (it normally takes about 4 days). Loads of travel agents will ge able to get it done for you if you don't want to go through the hassles of finding and getting to the Vietnamese embassy yourself. You'll need a couple of passport sized photos, so it can make it easier if you get some taken while at home and take them with you.

The Vietnamese embassy in Bangkok is pretty accessible. Just get the Sky Train to Phloen Chit station and head down Witthayu (Wireless) Road to number 83.

Get the vaccines, there important, you don't wanna be getting seriously ill in countries like Cambodia, hospital facilites are not that good, although Thailand is good and Vietnam not so bad. Travel insurance is advised too and is pretty cheap.

Anti malarials will be recommended to you, personally i did'nt take em just covered up and used Deet(still got bit a bit). I was ok but its really up to you.

BTW don't let any of the above put you off, its a great experience
